You customize your order by filling in the boxes provided with the following information, please double check to make sure they are correct: 1.Your loved one's name 2. Date of birth 3. Date of death and 4. the relationship(s) of your loved one to you and your family, for example he/she was a mother, father, wife, husband, grandparent, aunt, uncle, friend, etc. You may choose up to 5 relationships to be printed. We do not send out proofs as this item is very time sensitive, we inspect them for accuracy to ensure they are correct. Unfortunately, we are unable to add a photo of your loved one. We place 40-50 Forget Me Not seeds in each packet except in Connecticut, we would put wildflower seeds instead. These are favors not commercial garden packet!
